Montessori Enrollment Packet (Primary)

Here you will find every item you need for beginning TK or kindergarten with us!

Please click the underlined links below to print a pdf of each form, and be sure to make yourself a hard copy of the Parent Handbook to refer to throughout the year.

Before or on the first day that your child attends the program, please bring:

 

  1. $75 registration fee, if not paid at time of registration

  2. Cash or check for first month tuition (or record of Heartland fund submission)

  3. Cash or check for May tuition

  4. Parent Contract 

  5. Identification and Emergency Information

  6. Consent for Emergency Medical Treatment

  7. Child’s Preadmission Health History- Parent Report

  8. First-Aid Consent Form
